Major depressive disorder (MDD) is almost exclusively recurrent. The vast majority of patients who experience one episode of MDD will eventually experience at least one more episode during their lifetime. The recurrent nature of MDD increases the burden to both the individual and society. Hence, it is imperative that treatment strategies focus on achieving remission acutely, as well as maintaining of remission and preventing recurrence. The articles in this supplement are based on presentations and a dialogue among a group of experts who convened for a roundtable discussion on improving long-term outcomes with antidepressant therapy.
Improving long-term treatment of MDD begins with understanding the clinical course of recurrent depression and the ability to recognize those patients who are at greatest risk for recurrence. James H. Kocsis, MD, reviews the course of recurrent depression, emphasizing the tendency for it to progressively worsen. He also discusses patient characteristics and other risk factors for recurrence as well as current recommendations for long-term management of recurrent depression.
Although long-term antidepressant maintenance treatment studies are somewhat limited in number, they provide the evidence base that shapes existing guidelines for long-term management of recurrent depression. Michael E. Thase, MD, examines this evidence for the different classes of antidepressants. In addition, Thase reviews evidence for the efficacy of psychotherapy and discusses its potentially important role in long-term depression management.

A consensus conference on the reasons for the undertreatment of depression was organized by the National Depressive and Manic Depressive Association (NDMDA) on January 17–18,1996. The target audience included health policymakers, clinicians, patients and their families, and the public at large. Six key questions were addressed: (1) Is depression undertreated in the community and in the clinic? (2) What is the economic cost to society of depression? (3) What have been the efforts in the past to redress undertreatment and how successful have they been? (4) What are the reasons for the gap between our knowledge of the diagnosis and treatment of depression and actual treatment received in this country? (5) What can we do to narrow this gap? (6) What can we do immediately to narrow this gap?

This paper presents the design and science goals for the SkyMapper telescope. SkyMapper is a 1.3-m telescope featuring a 5.7-square-degree field-of-view Cassegrain imager commissioned for the Australian National University's Research School of Astronomy and Astrophysics. It is located at Siding Spring Observatory, Coonabarabran, NSW, Australia and will see first light in late 2007.
The imager possesses 16 384 × 16 384 0.5-arcsec pixels. The primary scientific goal of the facility is to perform the Southern Sky Survey, a six-colour and multi-epoch (four-hour, one-day, one-week, one-month and one-year sampling) photometric survey of the southerly 2π sr to g ∼23 mag. The survey will provide photometry to better than 3% global accuracy and astrometry to better than 50 milliarcsec. Data will be supplied to the community as part of the Virtual Observatory effort. The survey will take five years to complete.

Background
It is well established that the presence of prominent anxiety within depressive episodes portends poorer outcomes. Important questions remain as to which anxiety features are important to outcome and how sustained their prognostic effects are over time.
AimsTo examine the relative prognostic importance of specific anxiety features and to determine whether their effects persist over decades and apply to both unipolar and bipolar conditions.
MethodParticipants with unipolar (n = 476) or bipolar (n = 335) depressive disorders were intensively followed for a mean of 16.7 years (s.d. = 8.5).
ResultsThe number and severity of anxiety symptoms, but not the presence of pre-existing anxiety disorders, showed a robust and continuous relationship to the subsequent time spent in depressive episodes in both unipolar and bipolar depressive disorder. The strength of this relationship changed little over five successive 5-year periods.
ConclusionsThe severity of current anxiety symptoms within depressive episodes correlates strongly with the persistence of subsequent depressive symptoms and this relationship is stable over decades.

Background
Much remains unknown about the phenomenology of bipolar I disorder.
AimsTo determine the type of bipolar I mood episodes that occur over time, and their relative frequency.
MethodA total of 219 individuals with Research Diagnostic Criteria bipolar I disorder were prospectively followed for up to 25 years (median 20 years). Psychopathology was assessed with the Longitudinal Interval Follow-up Evaluation.
ResultsOverall, 1208 mood episodes were prospectively observed. The episodes were empirically classified as follows: major depression, 30.9% (n = 373); minor depression, 13.0% (n = 157); mania, 20.4% (n = 246); hypomania, 10.4% (n = 126); cycling, 17.3% (n = 210); cycling plus mixed state, 7.8% (n = 94); and mixed, 0.2% (n = 2).
ConclusionsCycling episodes constituted 25% of all episodes. Work groups revising ICD–10 and DSM–IV should add a category for bipolar I cycling episode.

The objective was to compare measures from dual-energy X-ray absorptiometry (DXA), bioelectrical impedance analysis (BIA) and anthropometry with a reference four-compartment model to estimate fat mass (FM) and fat-free mass (FFM) changes in overweight and obese women after a weight-loss programme. Forty-eight women (age 39.8 ± 5.8 years; weight 79·2 ± 11·8 kg; BMI 30·7 ± 3·6 kg/m2) were studied in an out-patient weight-loss programme, before and after the 16-month intervention. Women attended weekly meetings for the first 4 months, followed by monthly meetings from 4 to 12 months. Body composition variables were measured by the following techniques: DXA, anthropometry (waist circumference-based model; Antrform), BIA using Tanita (TBF-310) and Omron (BF300) and a reference four-compartment model. Body weight decreased significantly ( − 3·3 (sd 3·1) kg) across the intervention. At baseline and after the intervention, FM, percentage FM and FFM assessed by Antrform, Tanita, BF300 and DXA differed significantly from the reference method (P ≤ 0·001), with the exception of FFM assessed by Tanita (baseline P = 0·071 and after P = 0·007). DXA significantly overestimated the change in FM and percentage FM across weight loss ( − 4·5 v. − 3·3 kg; P < 0·001 and − 3·7 v. − 2·0 %; P < 0·001, respectively), while Antrform underestimated FM and percentage FM ( − 2·8 v. − 3·3 kg; P = 0·043 and − 1·1 v. − 2·0 %; P = 0·013) compared with the four-compartment model. Tanita and BF300 did not differ (P>0·05) from the reference model in any body composition variables. We conclude that these methods are widely used in clinical settings, but should not be applied interchangeably to detect changes in body composition. Furthermore, the several clinical methods were not accurate enough for tracking body composition changes in overweight and obese premenopausal women after a weight-loss programme.

Background
Cross-sectional data show that generalised anxiety disorder (GAD) is a chronic condition with episodes lasting much longer than the six-month minimum required by DSM–III–R and DSM–IV. Although GAD is chronic, little is known about factors influencing illness duration.
AimsTo investigate variables that influence the clinical course of GAD.
MethodA total of 167 patients with GAD participated in the Harvard–Brown Anxiety Research Program. Patients were assessed at intake and re-examined at six- to twelve-month intervals for five years. Kaplan–Meier curves were constructed to assess the likelihood of remission. Regression analysis was used to investigate factors predicting full or partial remission.
ResultsThe rate of remission was 0.38 after five years. Diminished likelihood of remission was associated with low overall life satisfaction, poor spousal or family relationships, a concurrent cluster B or C personality disorder and a low global assessment score.
ConclusionsFull or partial remissions were less likely to occur in patients with poor relationships and personality disorders. These patients should be given more intensive and possibly multi-modal therapy.

Background
The diagnostic category of generalised anxiety disorder (GAD) was originally intended to describe residual anxiety states. Over the years clinical criteria have been refined in an attempt to describe a unique diagnostic entity. Given these changes, little is known about the clinical course of this newly defined disorder. This study investigates the longitudinal course, including remission and relapse rates, for patients with DSM–III–R defined GAD.
MethodAnalysis of the 164 patients with GAD participating in the Harvard Anxiety Research Program. Patients were assessed with a structured clinical interview at intake and re-examined at six month intervals for two years and then annually for one to two years. Psychiatric Status Ratings were assigned at each interview point. Kaplan–Meier curves were constructed to assess likelihood of remission.
ResultsComorbidity was high, with panic disorder and social phobia as the most frequently found comorbid disorders. The likelihood of remission was 0.15 after one year and 0.25 after two years. The probability of becoming asymptomatic from all psychiatric symptoms was only 0.08.
ConclusionsThis prospective study confirms the chronicity associated with GAD and extends this finding to define the one and two year remission rates for the disorder. Likelihood of remission for GAD and any other comorbid condition after one year was half the annual remission rate for GAD alone.

Background
Dysthymia has been reconceptualised in recent years from a personality disorder to a chronic affective disorder. It is incorporated into both the DSM and ICD diagnostic systems. Method. The members of the WPA Dysthymia Working Group combined the results of their manual literature searches with a search using Medline.
ResultsAvailable data are summarised under the headings of classification, epidemiology, validity, comorbidity, course and outcome, pharmacotherapy and psychotherapy. The coexistence of major depressive disorder, constituting ‘double depression’ is of particular importance.
ConclusionsImproved knowledge of this disorder has led to a more positive approach to treatment, in which antidepressants can usefully be complemented by psychosocial measures. A high proportion of cases remain unrecognised in most populations, leading to prolonged morbidity and distress, much of which is now treatable.

The realisation that major depression is often both chronic and recurrent has slowly begun to change the way that depression is diagnosed and treated. In particular, the need for continuation and maintenance treatment is an issue that now deserves increased attention, especially with the availability of new classes of antidepressant treatments, which have excellent efficacy and more favourable side-effect profiles. Although the serious consequences of depressive disorders clearly indicate the need for effective and prompt intervention on the part of clinicians, the results of several studies indicate that patients with depression consistently receive no or low levels of antidepressant therapy. It is hoped that, through continued education of health care providers and patients about the consequences of depression, the issue of undertreatment of this serious illness will be resolved.
